Welcome to the Madison County, Indiana Health Department’s vital Records online ordering system. Requests will be processed Monday-Friday 8:00 AM - 4:00 PM. The Health Department processes orders within 2-4 business days, excluding weekends and holidays, once all required information is submitted.
We have birth and death records of those who were born or died in Madison County, Indiana. For all birth and death records outside of Madison County, Indiana, you may contact that county or the Indiana Department of Health in Indianapolis (IDOH) vital records at 317-233-2700. If the birth or death occurred in another State, please contact the State it occurred in.
REQUESTING A BIRTH OR DEATH CERTIFICATE
Birth Certificates
- A certified copy of your birth certificate is on file in the County / State in which you were born.
- People born in Madison County, Indiana after 1882 may have a birth certificate on file with the Health Department. Not all records were recorded between 1882 - 1907.
- If the birth was registered 4 or more years after birth prior to June 30th 2014, and after 1 year starting July 1st 2014, that record is a delayed registration. A delayed birth record can only be obtained from the Indiana Department of Health.
Death Certificates
- Certified copies of death certificates can be obtained for individuals that passed away in Madison County, Indiana. Death records start in 1882, not all deaths are recorded from 1882 - 1907.

Standard Shipping is through the US Postal Service. Please allow 7-10 business days for your order to be processed and the document delivered.
IMPORTANT - The ID provided must match delivery address or provide documentation proving mailing address with requestor’s name & address. Ex. Mortgage Statement; Lease Agreement; Utility Bill; Insurance statement, ect.
